How to appeal an Old Age Security decision

The form ISP-3134 is used to request a review of the decision on your application for Old Age Security Programs, this includes Old Age Security, Guaranteed Income Supplement (GIS), Allowance/Allowance for the Survivor (ALW/ALWS). This form does not apply to Canada Pension program.

A few reminders:

  1. Ensure to write down your SIN or your Client Identification Number on every pages of the form.
  2. Sign the Declaration in section 5.
  3.  Please do not send the original copy. Ensure all photocopies are certified before submitting.
  4.  You have 90 days from the date you received the decision letter from the pension processing office to file your request for reconsideration. If your submission is late, you may request an extension and provide an explanation of why you are requesting a longer period.

Once you have completed the form, you may mail it directly to the pension office (addresses below) or drop it off at your nearby Service Canada centre.
