Statement of Estimated Income form ISP-3041
If you have gone through a dramatic drop or stopped personal income in the year 2023, you may qualify for higher Guaranteed Income Supplements (GIS) by completing the Statement of Estimated form for the specific year that your income had dropped.

This form is not publicly available, unfortunately, therefore you will have to contact the pension call centre at 1-800-277-9914 to request them to mail the form to you.

If you need assistance filling up the ISP-3041 form, you may either contact pension call centre to have the agent complete the form with you on the phone or you can visit your local Service Canada centre with your previous years tax information. Your question is very specific to your personal scenario, it would be best that you contact pension call centre to have an agent to run through the number in the system to find out if you qualify for GIS. The GIS program runs from July to June of each year. Your GIS benefits for the upcoming cycle (July 2022 to June 2023) will be calculated based on your 2021 income. If it was below GIS threshold, you will be paid GIS. However, based on what you said that you will be collecting pension from Berlin starting August 2022, this income will be calculated against your 2022 income tax wich will affect your entitlement to next GIS payment cycle (July 2023 to June 2024).
This is just the basic general rule of GIS benefit.

September 9, 2022 12:13 pmI retired Aygust this year.i sent form 3041 in April because my taxable income this year ,2022 is only my cpp which is very low and from july my only income ( taxable and non-taxablr) is only my cpp.
i received payment in August but gis is base on my income year 2021.when service canada will respond to my 3041 form of considering my curent year income in determina gis payment? Thank you

As far as we know, the ISP-3041 form takes up to 6 months to process. So it may be at your best interest to contact pension call centre directly to inquire about the review status.

This form is used to help pension processing agent to estimate pensioner’s income from past or future year in order to calculate their GIS entitlement. It is best that you contact pension call centre first to find out if this form is right for you.

The form ISP-3041 (Statement of Estimated Income) is not a public form and must be sent directly from pension processing office. You may only be able to request this form by calling the pension call centre at 1-800-277-9914.

If the pension is asked to complete the form, it is surely at their best interest to complete it fully. If the pensioner is going to retire within the current GIS benefit period (July 2022 to June 2023), then yes, the pension must list all potential income they will going to receive for the remaining year to help pension processing agent to calculate their benefits.

As far as we know, this the Estimated Income form only applied to pension applicant under very specific circumstantces. Therefore, it is up to the processing agent to decide if it is necessary to apply this form. This is why it is not a publicly available form.

The reason to complete a Statement of Estimated income form is that the processing office needs to calculate your OAS/GIS benefits in the past, current and future period. It is a case by case issue. I would recommend you to contact pension call centre to have an agent to explain to you which period is this form applies to.

The Statement of Estimated Income form is not publicly available, you have to contact pension call centre to have them mailed to you. Before they mail, they will do an assessment on the phone with you to ensure it is needed for you. Otherwise, you can visit your closest Service Canada Centre to pick up a copy as well.

My question, does the benefit amount go back to receipt of application or when it has been processed. is there back pay from January 1, 2023 – date of retirement when income changed.

And what about RRSP deductions planned for that year ? Yhey are also given in advance ?

Sometimes, the pension processing office will send you the ISP-3041 form because they want to make sure the pension benefit is accurately reflecting your current life scenario. E.g. retirement. To ensure if this form is truly coming from pension office, you may call their call centre at 1-800-277-9914 and the agent will be able to assist you.

My only income for 2024 will be my OAS which is clawed back 514.00 because of a lump sum in 2023.
I have had Service Canada send me the 3041 form and my local office filled it for me. I am wondering do I still fill out a
1213 OAS to send to Revenue Canada to estimate income for current year for my OAS or will the Gis make up the difference

May 25, 2024 8:58 amWhy don’t you provide form SC ISP-3041 fillable PDF online. It’s much easier to fill out on the PC and print it out. It’s very frustrating not being able to find several forms online using your website. Some form you need to send a request by mail to them, what’s up with that ? Why make thing difficult ? Maybe the person responsible should be replaced with someone who actually cares !
