Due to unforeseen circumstances resulted from the COVID-19 outbreak, many school’s registrar’s office may be closed or have limited in-person services. This might result into complications for students who are receiving CPP Child’s Benefit to have their Declaration of Attendance at School or University (ISP-1401) form completed and signed by the authorized school official.
As for the fall 2020 semester, Child’s Benefit applications can submit an attestation of school attendance in lieu of the ISP1401 form.
You may send the following documents to prove your enrollment:
- Email from school official
- A printed letter from school’s student online service portal
The attestation must include all mandatory information such as student’s:
- name
- student ID
- program of study
- name of the school
- address of school
- school’s official electronic signature
